Client Information & Forms

This is the page for forms, questionnaires, surveys, clinic and professional policies we adhere to in order to provide you with professional and 'best practice' therapy to meet your needs.

Take an opportunity to provide us your feedback.

We acknowledge the Traditional Owners and Custodians of the lands on which we live and work. We pay our respects to Indigenous Elders past, present and emerging. Sovereignty has never been ceded. It always was and always will be Aboriginal land.

We are committed to providing an inclusive and safe environment for all people, regardless of race, religion, language, background, age, ability, gender identity or sexual orientation. Everyone deserves respect and the opportunity for a healthy life.

Keeping Children Safe

At All About Kids, the safety and wellbeing of every child and young person we work with comes first. We follow the Queensland Child Safe Standards and the Universal Principle under the Child Safe Organisations Act 2024.

What this means for your family:

Our clinicians and team members complete Blue Card and NDIS Worker Screening checks in line with NDIS and Queensland regulatory requirements. They also sign our Child Safe Code of Conduct and are trained to recognise and respond to concerns about a child’s safety.

We tell children, in a way that suits their age, that they have the right to feel safe and who they can tell if something feels wrong.

If we are concerned that a child may be at risk of harm, we must report it to Child Safety or the police. We may not be able to tell you first if doing so could put the child at risk.

You can raise a concern about your child’s safety, or about any member of our team, with us at any time. You can also contact the Queensland Family and Child Commission directly.

Supervision

You are responsible for supervising your child before and after their session, including in our waiting areas. A parent or guardian stays on site for the whole session, unless we have agreed in writing that your young person may attend on their own.

Confidentiality

Confidentiality between a young person and their clinician is essential for therapy to work, and our
clinicians take it very seriously. Information shared in sessions is only passed on with permission,
except where:

• someone’s safety is at serious risk;
• we are concerned a child may be at risk of harm, which we must report;
• the law requires it, for example a court subpoena.

Your clinician will always encourage a young person to talk with their parent or guardian.
For children and young people, we need a parent or guardian’s consent for assessments, therapy, and sharing information with other professionals such as teachers or therapists. If there are parenting orders, please give us a copy. If you have a Medicare referral, your clinician must give your referring GP or paediatrician information about diagnosis and treatment.

Information Privacy

At All About Kids, we know that when you share information about your child and your family, you are placing your trust in us. We take that trust seriously. Everything you tell us — from contact details to the things you share in sessions — is treated with care and respect, and handled in line with the Privacy Act 1988 (Cth) and the Australian Privacy Principles.

We only collect the information we need to provide a safe and effective service.

Only the people involved in your care can see your information, and we share it with others—such as your GP or school—only with your consent, unless the law requires it, for example, to keep a child safe.

We store your records securely and keep them for as long as the law requires.

If you have a question or concern about privacy, please contact us at [email protected].

Assessment and Therapeutic Reports

All About Kids is a counselling and therapeutic intervention clinic. Clinicians conduct clinical, educational and developmental assessments and provide intervention accordingly. All About Kids clinicians can provide assessment and therapy reports as requested by clients for a range of requirements, including providing information to schools, medical practitioners, parents, and other therapists. Clinicians are also available for teleconferencing or school visits in order to share or discuss pertinent information and intervention strategies. An appropriate fee may be charged for the provision of such services.

Court and Legal Reports

We do not provide legal reports, including family reports or assessments for the Family Court or other legal matters. If you need your child’s information for court, we can prepare a factual therapy report summary of their assessment, therapy, and progress. This is not a legal report or an expert opinion. We can refer you to psychologists who provide family reports.

Cancellation Policy

Our sessions are 50 minutes in duration. If appointments are not attended, we cannot fill the session time with other clients on our wait list. Therefore, we have a Cancellation and Non-Attendance Policy.
